A Geriatric Care Manager or one of our highly trained staff will meet with your loved one and family at their home, hospital, rehab or current living setting. Our comprehensive patient-centered approach takes a 360° perspective in determining your loved one’s needs. The goals of the assessment are:

° Gather Health and Medical History
° Identify Personal Care Needs
° Determine the appropriate level of care and determine the days and hours for home care services
° Identify all other needs of the client, home cleaning, transportation, other supportive services
° Get to know the client on a personal level to match the caregiver
